How to use these numbers

  1. Find the service level on your bill. Look for codes like A0427 (ALS1 emergency) or A0429 (BLS emergency).
  2. Check the mileage. Your bill should charge for loaded miles only, under code A0425 for ground rides.
  3. Compare. On Original Medicare, the ambulance company can't collect more than the Medicare-approved amount. You owe 20% after the Part B deductible. With private insurance or no insurance, the Medicare number is a benchmark you can cite when you negotiate.

Frequently asked questions

›What does this tool show?

It shows the amount Medicare allows (the approved amount) for each ambulance service level in the ZIP code where you were picked up. It uses CMS CY2026 Ambulance Fee Schedule; ZIP code to carrier locality file rev. 2026-08-13. Mileage is calculated for the number of loaded miles you enter.

›Is the Medicare rate what my ride should cost?

Not necessarily. It's a reference point. Under Original Medicare it's the most an ambulance company can collect for a covered ride, because ambulance suppliers must accept Medicare's approved amount. Private insurers usually pay more, and ambulance companies often charge much more than either. Comparing your bill with the Medicare amount shows how far above that baseline you were charged.

›Why does my ZIP code ask for a ZIP+4?

Medicare splits some ZIP codes between different rate areas, or between urban and rural status, based on the 4-digit ZIP+4 extension. If you don't know it, the tool shows every possibility.

›What are loaded miles?

Loaded miles are the miles driven with you on board, from the pickup point to the hospital. They don't include the trip to reach you.
